Evaluation is a vital element of any health education program.  Therefore, NCPRS is committed to providing organizations, teachers, administrators, and health education coordinators with the tools to evaluate the program effectively.  If you are interested in finding out how Stay on Track will work in your community/organization please let us know.  Our Evaluation Team can help you identify and track how adolescents that participated in the Stay on Track program have experienced a change in knowledge, attitude, and intent.  Some of the evaluation tools available include:

Pre/Post-Program Scantron Survey

  • Surveys that we are currently using to evaluate the Stay on Track program can be modified to fit your individual needs.
  • Surveys can be customized via the web, server, or ISP.

  • Implementation Checklist(s)

    In order to track the fidelity of the program, you can order implementation checklist(s) that will allow the Stay on Track implementers to track adherence to the Stay on Track program. 

    Surveymonkey

    If you are in need of other forms of assessments, our Evaluation Team can create a survey for your organization via surveymonkey.

    Additionally, NCPRS evaluation specialists can assist with the following resources:

  • Authenticate respondents
  • Branch questions online
  • Create multiple of single page surveys
  • Administer surveys via email, LAN/WAN, or standalone kiosk
  • Provide a quantitative analysis of the program
  • Exported data into Excel, SPSS, CSV, Crystal, or Access file
